【内容提要】新药临床试验专业性强、风险高。在其组织运行上，中国医院普遍采取了一种“组织内外包”的混合模式，将试验中的部分工作外包给第三方公司，但要求公司派遣协调员嵌入医院的场域空间和组织框架开展工作。这一模式回应了医院面临的商业、专业、行政三重制度逻辑的要求。首先，市场化和科层化并存的用工结构保障了医院利润最大化；其次，空间嵌入特征满足了临床试验高技术专业性和高程序专业性的双重要求；最后，“内外有别”的身份区隔设计转移了风险。“组织内外包”是理解新药研发过程的重要概念和视角。

Organizational Inside-Outsourcing: A Study on the Organizational Model of New Drug Clinical Trials

Abstract: New drug clinical trials are highly specialized and risky. In terms of organizational operation, Chinese hospitals have generally adopted a hybrid model of ‘organizational inside-outsourcing.’ In this model, trial work is outsourced to third-party companies, yet the dispatched clinical research coordinators remain embedded within the hospitals’ spatial and organizational framework. This model responds to the demands of the commercial, professional, and administrative logics that hospitals face. Specifically, the dual employment structure, combining market-based and bureaucratic mechanisms, maximizes hospital benefits. The feature of spatial embeddedness meets the dual requirements of high technical and high procedural professionalism. Furthermore, the ‘inside-outside’ distinction mechanism facilitates risk transference. Thus, this model serves as a crucial analytical lens for understanding the social process of new drug development.
